Our team also supports the Certificate of Need (CON) process by assisting with the collection of impactful letters of support. In addition, we help generate referrals directly from the community to strengthen their network and drive meaningful growth.

We help Home Health Agencies boost their visibility within the community by actively sharing information about their services and building strong connections with local healthcare providers and organizations. Our team works to generate high-quality referrals from the community, ensuring your agency stays top-of-mind for those in need of reliable, compassionate care.

We support Private Duty companies by actively networking on their behalf and organizing luncheon events with key agencies that can open doors to patient referrals. These strategic connections often include large hospice companies and home health agencies, helping private duty providers build strong referral pipelines and expand their reach within the community.

We assist Skilled Nursing Facilities (SNFs) by promoting the facility within the community—especially when there are open beds—helping to maintain steady admissions. Our team also supports the Social Services department by helping place patients in assisted living communities, home health agencies, and hospice companies, including those challenging insurance cases that not every provider is willing to accept.

We help Assisted Living Facilities maintain consistency and full occupancy by actively promoting the community and connecting them with potential residents. In addition, we ensure that residents receive the extra care they need—whether through home health or hospice services, if eligible. We also coordinate with select Skilled Nursing Facilities to provide temporary skilled care that cannot be performed in the ALF, with the goal of ensuring the patient returns to the assisted living community once stabilized.

We assist hospitals with a smooth and efficient discharge process by helping place patients in the right Skilled Nursing Facility (SNF), Assisted Living Facility (ALF), hospice company, or home health agency that best fits their needs. We also work with clinics and community doctors to help drive more patients to their offices while ensuring their current patients are connected to the appropriate level of care—whether it's home health, hospice, SNF, or ALF services.
